A copy of the participant's birth certificate is required at registration. Physical forms will be distributed at registration. To have your registration form completed when you arrive, download the form here.
ADDITIONAL INFORMATION FOR ALL FOOTBALL AND CHEERLEADING PARTICIPANTS. Participants are not eligible to participate in practice or games/competitions if a copy of their birth certificate and the completed physical form are not on file with the CVMFA Football/Cheerleading Registrars. There are no exceptions to this rule. Physicals MUST be for the current year and the forms must be properly completed with all signatures. NO SUBSTITUTE FORMS WILL BE ACCEPTED. _____________________________________________
